Most of the human body has the same anatomical structure with 01 uterus (as well as 01 heart, 01 liver, 01 spleen...), 02 kidneys (as well as 02 lungs, 02 testes, 02 chambers) egg..). However, in very rare cases, some people have congenital anatomical abnormalities that cause them to have differences in internal body parts that we cannot detect on the outside. , only when doing physical examination and doing paraclinical tests will it be discovered. The new case recorded in the emergency room of Binh Dinh hospital is one of such rare cases, the timely recognition of these abnormalities contributes greatly to the diagnosis and management of patients due to the disease. These anatomical abnormalities are often accompanied by dangerous health complications.

Specifically, at 22:00 on July 27, 2022, the Emergency Room (Emergency Department) received a 30-year-old female patient from Binh Dinh in Binh Dinh with severe abdominal pain in the lower abdomen. The left iliac fossa and abdomen through examination, laboratory examination and history taking, the emergency physician noted that the patient had signs of severe infection with the focal point of suspicion from the abdomen, clear and subclinical abdominal wall reaction. The white blood cell count was very high (a sign of a serious infection) and the most surprising thing was that the patient had 2 uteruses, also known as bisected uterus (Uterus Didephys) and moreover, this patient only had 2 uteruses. Only 01 kidney on the Right (Ispilateral renal agenesis), these two abnormalities have suggested to the emergency doctor the possibility that the patient has a very rare genetic syndrome in Vietnam called HERLYN-WERNER-WUNDERLICH , this syndrome is characterized by 03 typical abnormalities of congenital body anatomy, that is: also split uterus (02 uterus), 01 aplastic kidney (only one developing kidney remains) and secondary abnormality. 03 is holding the way Menstruation of one half of the uterus (Blind hemivagina). From this finding, the emergency doctor said that there is a high probability that the patient will have acute and dangerous complications in obstetrics and gynecology as well as surgery (such as stillbirth, ovarian tumor, ruptured abdominal tumour, causing peritonitis). , uterine necrosis, intra-abdominal infection, mesenteric lymphadenopathy, etc.), therefore, the patient quickly underwent additional laboratory tests and consulted with Obstetricians and Surgeons. After the consultation, the patient was diagnosed with peritonitis due to ruptured abdominal tumor/HERLYN-WERNER-WUNDERLICH syndrome and was promptly treated with aggressive surgery at the Department of Surgery, Binh Dinh Hospital. After surgery and treatment, the patient's health recovered well and was discharged

Through this special and rare case recorded at the Emergency Department-Binh Dinh Hospital, it shows that periodic health check-ups and screenings are essential so that we can detect health abnormalities early to early intervention.
